Skilled Nursing (SN) refers to providing skilled services to a member in the home for the purpose of restoring and maintaining his or her maximal level of function and health. These services are rendered in lieu of hospitalization, confinement in an extended care facility, or going outside of the home for the service.
COMPANION (COMP)
Companion care is primarily emotional support and companionship for seniors who are generally healthy and who want to remain independent at home. However, it can also include a range of non-medical services that help make a senior's life more manageable.
RESPITE CARE
Respite programs provide planned short-term and time-limited breaks for families and other unpaid care givers of children with a developmental delay and adults with an intellectual disability in order to support and maintain the primary care giving relationship. Respite also provides a positive experience for the person receiving care.
HOMEMAKING (HMK)
Homemaker services provide assistance to persons who are unable to perform day to day household duties and have no one available to assist them. It includes services like light housekeeping, laundry, limited personal care, grocery shopping, meal preparation, and shopping.
Personal Care Assistance (PCA), Home Health Aide, programs involve attending to the physical needs of people who are disabled or otherwise unable to take care of themselves, including tasks such as bathing, management of bodily functions, and cooking.
PHYSICAL THERAPY
Physical Therapists (PT) diagnose and treat individuals of all ages who have medical problems or other health related conditions, illnesses, or injuries that limit their ablilities to move and perform functional activities in their daily lives. Therapy is performed by a therapist sometimes services are provided by an Assistant (PTA) acting under their direction.
OCCUPATIONAL THERAPY
Occupational Therapists apply their specific knowledge to enable people to engage in activities of daily living that have specific meaning and value. Develop, improve, sustain, or restore independence to any person who has an injury, illness, disability or psychological dysfunction.
SOCIAL WORK
Social work is a professional and academic disciplline that seeks to improve the quality of life and subjective well being of individuals, families, couples, groups, and communities through research, policy, community organizing, direct practice, crisis intervention and teaching for the benefit of those affected by social disadvantages.
IV THERAPY
Intravenous Therapy at Home affords our patients the special opportunity to receive intravenous medication or fluids at home. The goal of providing this service being to give patients independence in their care and decrease the need for patients to stay in a hospital.
